Hi Angie, sorry you're a bit stuck
Have another squint at the photos, I did all the beads different colours so you can see what's going on.
You are adding one bead (the yellow ones in fig 5) and two beads (the bronze ones in fig 5) in alternate stitches to make the row increase and spiral; you put one in between the two beads of the pair, come out of the second bead of the pair, add two beads, go into the first bead of the next pair, add one bead, come out of the second bead, add two beads... and so on.
You go through each bead of the previous row (the green ones in fig 5) without missing any out.
Hope that helps - the next row is easier, you are only adding one bead between each bead of the previous row, and again you're going through each bead of the previous row and not missing any.
Shout again if you get stuck - I can dig out some bigger photos if you need them!
(Oh, and don't worry if you miss the odd stitch, it'll still spiral as long as you've added more beads in each row, there isn't a right and wrong way to make a peyote spiral and you may find you've invented your own way which is easier than mine... that's fine!)
